ÖZET
İnvitro fertilizasyon (IVF) sonucu oluşan ikiz gebeliklerde spontan konsepsiyon sonucu oluşan ikiz gebeliklere göre obstetrik riskler ve olumsuz perinatal sonuçların arttığı yönünde kanıtlar mevcuttur. Bu çalışmanın amacı spontan ve IVF sonrası meydana gelen ikiz gebeliklerde obstetrik ve perinatal sonuçların karşılaştırılmasıdır. Gereç ve Yöntemler: Ocak 2010-Ocak 2020 tarihleri arasında antenatal takipleri ve doğumları merkezimizde gerçekleşen tüm DDA ikiz gebelikler çalışmaya dahil edildi. Obstetrik ve perinatal sonuçlar konsepsiyon şekline göre karşılaştırıldı. Veriler SPSS 24.0 programı ile değerlendirildi; p<0,05 olan değerler istatistiksel olarak anlamlı kabul edildi. Bulgular: Çalışmaya ikiz gebeliği olan toplam 330 hasta dahil edildi. Tüm ikiz gebeliklerden 118’i (%35,75) IVF tedavisi sonucu ve 212’si (%64,24) spontan konsepsiyon sonucu oluşan ikiz gebelikti. IVF ikiz gebeliklerinde gestasyonel diyabet (GDM) gelişme riskinin spontan ikiz gebeliklere göre anlamlı düzeyde daha yüksek olduğu bulundu. Fetal yapısal anomaliler IVF grubunda anlamlı olarak daha yüksekti. Gebeliğin hipertansif hastalıkları (GHT) ve erken membran rüptürü (EMR) oranları her iki grupta benzerdi. İki grup arasında doğum şekli, ortalama doğum haftası ve doğum ağırlıkları arasında anlamlı fark saptanmadı. Sonuç: IVF ikiz gebeliklerinde GDM riskinde artış olmakla birlikte perinatal sonuçlar açısından spontan ikiz gebelikler ile karşılaştırıldığında fark saptanmadı. IVF tedavilerinin fetal anomali riski üzerine etkisinin değerlendirildiği daha geniş çalışmalara ihtiyaç vardır.
ABSTRACT
There is conflicting data on whether adverse obstetric and perinatal outcomes are increased in twin pregnancies conceived with IVF treatment compared to spontaneous twin pregnancies. The aim of this study is to compare obstetric and perinatal outcomes in twin pregnancies occurring spontaneously and after IVF treatment. Material and Method: All DDA twin pregnancies whose antenatal follow-ups and deliveries were carried out in our center between January 2010 to January 2020 were included in the study. Obstetric and perinatal outcomes were compared between twin pregnancies according to the mode of conception. The data were evaluated with SPSS 24.0 software; p value<0,05 was considered statistically significant. Results: A total of 330 patients with twin pregnancies were included in the study. Of all twin pregnancies, 118 (35.75%) were twin pregnancies resulting from IVF treatment, and 212 (64.24%) have occurred after spontaneous conception. The risk of gestational diabetes (GDM) was significantly higher in IVF twin pregnancies than spontaneous twin pregnancies. Fetal structural anomalies were significantly higher in the IVF group. Gestational hypertensive disorders (GHT) and premature rupture of membranes (PROM) rates were similar in both groups. Conclusion: There was no significant difference between the two groups in terms of the type of delivery, gestational week at birth, and birth weight. Although there is an increased risk of GDM in IVF twin pregnancies, our results showed that perinatal results were similar to spontaneous twin pregnancies. Further studies evaluating the effect of IVF treatments on fetal anomaly risk are needed.
